What is Retatrutide?

Three receptors, one molecule: retatrutide is an investigational agonist of GIP, GLP-1 and glucagon receptors. This article explains its mechanism and clinical evidence in context.

How is retatrutide built?

Retatrutide is a synthetic peptide developed from a glucagon-based peptide and acts as an agonist at three receptors: GIP, GLP-1 and glucagon. It is therefore described as a triple agonist. Tirzepatide, by comparison, targets GIP and GLP-1 receptors.

Structural modifications, including a fatty-acid side chain, extend exposure compared with the unmodified parent peptide. Suitable experimental conditions depend on the specific research protocol.

What happens with three keys at once?

Retatrutide activates three receptor systems. GLP-1 and GIP are incretin pathways; retatrutide additionally targets the glucagon receptor. The combined effects are being studied in preclinical and clinical research.

Activation of these pathways can affect several physiological processes, including appetite, glucose homeostasis and energy expenditure. The magnitude and direction of effects depend on dose, model and study population.

Observed overall effects cannot be assigned to individual receptors without suitable comparator arms. Studies therefore assess both efficacy endpoints and safety, while parts of the mechanistic explanation remain under investigation.

What researchers see in studies so far

A randomized, placebo-controlled 48-week phase 2 trial observed dose-dependent changes versus placebo. It was not a head-to-head trial against tirzepatide, so cross-trial comparisons cannot establish superiority. Other studies have also examined liver fat.

In June 2026, TRANSCEND-T2D-1, a completed randomized phase 3 trial in adults with type 2 diabetes, was published; other clinical programmes continue. Clinical investigational products and the research reagents sold here are not legally or qualitatively equivalent.
